Reading your post, I couldn't help noticing the 'elephant in the room'. Opiates! If your pain is still persisting, you could find an understanding doctor who is willing to prescribe you a low-dose, slow release opiate like Oxycontin. This is by no means a 'total' solution, but it will assist in reducing your pain.
